Permission Slip for 2006-2007 Diversity Film Series

My son/daughter, __________________________, has my permission to view R-rated films at Queen Anne School’s 2006-2007 Diversity Film Series. While these are all highly acclaimed films, I am aware that they may include violence, language, adult situations, and/or sexual content that caused them to receive their rating.
